Hywel Dda - Our Big NHS Change

Hywel Dda University Health Board is proposing changes to the way health services are organised and we would like to hear your views or any new ideas. We have an ambition for our local NHS and a once-in-a-lifetime opportunity to work with you to make it better for everyone. We want to improve health and well-being for all and provide high quality care and a good patient experience. We believe we need to provide more health and care in the community and change the way our hospitals provide care. Our doctors, nurses and other healthcare professionals say we must change to meet the growing needs of our population and the challenges facing the NHS, such as recruiting the workforce needed.

 
Our consultation document outlines our proposals for community care and three proposals for hospitals which we think are safe, viable and offer an improvement on what we currently have. It is important that you read our consultation document, or see our alternative versions on our consultation webpage including an Animation or Easy Read, before providing your feedback. There are many ways you can share your feedback, including using this questionnaire to give us your views on the proposals. The documents, ways to share your feedback and questionnaire can be found online at www.hywelddahb.wales.nhs.uk/Hddchange or by requesting a copy from the Health Board.

 

All questionnaires should be completed by Thursday 12 July 2018.
